Complete the Freddy L. Jackson Citizenship Award application.

Submit the application and the acceptance letter from the accredited and approved technical, two-year, or four-year educational institution within the United States which you plan to attend in the Fall of 2024.

Optional: Applicants are welcome to also submit a student resume.
Submission Process
OPTION 1:
Complete and submit application online and then email remaining required/optional documentation to:

Please include your name in the subject line.
OPTION 2:
Email package including completed application (.pdf, .doc, .docx,) and required/optional documentation to:

Please include your name on the subject line.
Complete application package must be received by April 26, 2024 no later than 11:59 PM EST.

Late submissions will not be considered.

It is the responsibility of the applicant to ensure all required materials are submitted on time.
Notification

Selection Committee will notify selected recipient and West Haven H.S. Guidance Department by May 3, 2024.
