Instructions

  1. Complete the Freddy L. Jackson Citizenship Award application.

  2. Submit the application and the acceptance letter from the accredited and approved technical, two-year, or four-year educational institution within the United States which you plan to attend in the Fall of 2024.

  3. Optional: Applicants are welcome to also submit a student resume.

Submission Process

OPTION 1:  

Complete and submit application online and then email remaining required/optional documentation to:

Please include your name in the subject line.

OPTION 2:

Email package including completed application (.pdf, .doc, .docx,) and required/optional documentation to:

FLJCitizenshipAward@gmail.com

Subject: FLJ Citizenship Award Application

Please include your name on the subject line.

Complete application package must be received by April 26, 2024 no later than 11:59 PM EST.

  • Late submissions will not be considered. 

  • It is the responsibility of the applicant to ensure all required materials are submitted on time.

Notification

  • Selection Committee will notify selected recipient and West Haven H.S. Guidance Department by May 3, 2024.
